The Healthy Children and Youth (HCY) program
About The HCY Program
The Healthy Children and Youth (HCY) program is designed to help secure care for disabled youth across Missouri. It provides service coordination and authorization for services that are deemed medically necessary for people less than 21 years of age.
This includes home visits and resources that can ensure children can remain safely in their home with their families.
Eligibility:
The participant must:
* Be a Missouri resident
* Be birth to age 21
* Need medically necessary services
* Be enrolled in MO HealthNet

In-Home personal care assistance with ADL's and IADL's
Administration of medical services from our personal care assistance and professional nursing staff
Transportation
Meal planning/prepping
